Appointment Information

To make an appointment, please call (704) 403-1800 during normal office hours. New patients need to have their primary care physician call to schedule their first appointment with the clinic.

First Visit
Each new patient is asked to print and complete the new patient forms.

Please bring your insurance card and any prescription medications you are currently taking. Please arrive 15 minutes before your scheduled appointment to complete registration and other paperwork.

Emergencies
Our physicians are on-call 24 hours a day to offer help to patients in need. If you have an urgent problem, after hours or on a holiday, please call CMC - NorthEast at (704) 403-3000.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment. There will be a $50 fee for all appointments for which the patient does not attend without providing advanced cancellation or rescheduling.
